About the Role

Job Summary The post holder will provide clinical pharmacy and medicines optimisation support for older adults living

with frailty across care homes and the wider Primary Care Network. Working as a core member of the multidisciplinary

frailty team, the role supports proactive, personalised care to improve outcomes, reduce medicines-related harm and

prevent avoidable hospital admissions. Key Responsibilities Provide expert advice and support on prescribing and

medicines-related issues for patients with moderate to severe frailty. Undertake comprehensive and structured medication

reviews, including polypharmacy management and deprescribing where clinically appropriate. Work collaboratively with
