Clinical Pharmacist for Patient Safety
About the Role
Job Purpose The post holder will champion patient safety and good prescribing throughout the organization. They will
identify appropriate clinical searches to identify patients who warrant a review or intervention related to prescribing.
Amongst others, this will include follow up reviews or blood tests after medications have been newly started or periodic
reviews for continuing medication. Having identified these searches, the post holder will run reports to identify
relevant patients regularly. They will take steps to ensure that the appropriate review or intervention happens, where
appropriate completing the review themselves. The post holder will work with the wider clinical team to identify
