About the Role

The following are the core responsibilities of the EHUB clinical pharmacist. There may be on occasion, a requirement to

carry out other tasks, this will be dependent upon factors such as workload and staffing levels: Clinical

Responsibilities: a. Practice as an independent prescriber working within scope of practice b. Manage online

consultation requests routed to pharmacy through the EHUB c. Perform telephone consultations within scope of practice d.

Undertake structured and problem focused medication reviews e. Manage medication related requests for long term

conditions within scope of practice f. Support medicines optimisation and cost effective prescribing g. Support
