About the Role

Job Description Clinical Pharmacist (CP) Job Overview The Clinical Pharmacist will work as an integral member of the

multidisciplinary team across the Primary Care Network (PCN) to improve patient health outcomes, particularly for those

with long-term conditions. The role will focus on optimising medicines use, supporting safe and cost-effective

prescribing, and enhancing communication between patients, practices, and wider healthcare providers. The postholder

will provide specialist pharmaceutical expertise and evidence-based guidance to clinicians and patients, support

medication safety initiatives, and contribute to the achievement of QOF and IIF targets. They will either hold, or be
