About the Role
Key Responsibilities1. Patient-Facing Clinical Work Conduct structured medication reviews (SMRs) Manage long-term
conditions (e.g. hypertension, diabetes, asthma, COPD) Independent prescribing within scope of competence Manage acute
minor illnesses (if qualified) Undertake polypharmacy reviews, especially in elderly or frail patients Deprescribing
where appropriate 2. Medicines Optimisation & Safety Reconcile medicines following hospital discharge Review high-risk
medications (e.g. DOACs, methotrexate, lithium) Monitor blood tests and therapeutic drug levels Implement local and
national prescribing guidance (e.g. National Institute for Health and Care Excellence (NICE)) Lead audits to improve
