About the Role
Participating in the structured rotational programme, to include aseptic and community services across the sites as
necessary participating in the Foundation Level competency framework for Band 6 pharmacists Providing medicines
management, medicines optimisation and medicines reconciliation services to help achieve department priorities and
targets on a designated ward or group of wards as decided by the Lead Pharmacist. helping integrate role of medicines
reconciliation into the existing medicines management framework. helping implement local policies and guidelines on good
prescribing practice. influencing local practice to ensure safe medication use, and to ensure that their own practice
