About the Role

Key Responsibilities Clinical Responsibilities (ARRS-aligned) Conduct structured medication reviews (SMRs) for patients

with complex needs, including: Polypharmacy Frailty Long-term conditions Care home residents Provide clinical advice on

prescribing, medicines optimisation, and deprescribing. Manage medication-related queries from patients and clinical

staff. Support chronic disease management clinics in line with agreed protocols. Undertake medication reconciliation

following hospital discharge. Identify and manage medicines-related safety issues and risks. Prescribe medicines

independently (where appropriately qualified and authorised). Medicines Optimisation & Safety Support safe, effective,
