Clinical Sister / Charge Nurse - SMART

About the Role

Members of SMART work autonomously, therefore we are looking for someone with excellent evidence based experience in the

care of acute patients. Specific training will be provided to facilitate the pathways covered by SMART in the community.

Confidence, good communication skills and the ability to make sound decisions is vital. The Clinical Sister / Charge

Nurse must also be able to liaise with all members of the multidisciplinary team to coordinate safe effective and timely

care practices within the SMART service. The successful post holders main responsibility will be the day to day care of

patients on SMART h@h virtual ward, and also ensuring that all patients referred are triaged and prepared safely to
