About the Role
JOB DESCRIPTION
The H@H service primary focus is on maintaining older people at home in the event of a crisis or unexpected
deterioration in health. It aims to reduce length of stay for inpatient admissions, prevent hospital admissions and
facilitate early discharge. The post holder will function fully as a member of the H@H Multi-disciplinary Team and will
be required to participate in all multi-disciplinary meetings, share decision making, advise on specific Nursing
requirements and complete patient held and shared files for all patients and carers in a person centred manner. Please
