About the role
Co-ACTION is a five-year NIHR-funded research programme developing and testing a personalised, multi-sectoral support
model for people living with multiple long-term conditions (MLTCs).
The programme takes a holistic, bio-psychosocial approach, combining social and self-management support to improve
wellbeing and reduce inequalities. The Co-ACTION intervention is being evaluated through a mixed-methods trial across
several sites in England.
