Line Manager Location: Meridian Health Group - Community Team Job Summary The Community Advanced Clinical Practitioner

(ACP) will provide expert, autonomous clinical care to patients in care homes and those who are housebound. The

postholder will use advanced clinical skills, diagnostic expertise, and independent prescribing to assess, manage, and

review patients with complex health needs. They will play a key role in supporting proactive, person-centred care,

reducing avoidable hospital admissions, and improving quality of life for vulnerable populations. Key Responsibilities
