Community and Hospital Support Worker - Fixed Term

About the Role

We’re looking for someone to work on in our Hospital Discharge team. You’ll be working closely with integrated discharge

hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their

own homes in the community.

Please note

We will reimburse mileage and parking expenses for any travel undertaken as part of your role.
