An exciting opportunity has arisen for a Community Care Assistant to join the Assessment and Review Team.

Successful applicants will be supported by a Team Manager and will work in collaboration with colleagues from other

disciplines within the team which comprises Occupational Therapists, Social Workers, Community Care Coordinators and

technology staff.
