About the Role

This is an exciting opportunity to join a multidisciplinary team in the North Lancashire Community. The integrated

children's community nursing and therapy team is made up of CCN team, CDC, children's hearing service, occupational

therapy, physiotherapy and Speech and Language Therapy.

You will work as part of this wider team and will be responsible for provision of specialised assessment, treatment and

management of babies, children and adolescents who present with a wide variety of conditions. These include chronic,

complex, disabling and life limiting conditions.
