About the Role

An opportunity has arisen within South East Essex Dementia & Older Adults Community Mental Health Service for a

Registered or newly qualified Band 5 Mental Health Nurse.

The successful candidate would be working with the Care Home Team which supports and assists in assessment of needs for

individuals, supports diagnosis, appropriate on-going review and care planning for individuals in the residential care

settings for those with a diagnosis of dementia or suspected dementia.

The post holder will work with a range of professionals and link with other services within South East Essex. This
