About the Role
We are looking for a highly motivated Diabetes Nurse Specialist, who is passionate about diabetes ready to make a major
contribution to the care of people in the community with diabetes.
The post holder will be responsible, as part of a multidisciplinary clinical team, for supporting the care of people
with diabetes at their GP surgery.
The post holder will ensure that people receive the necessary care and treatment to best manage their long-term
condition. This will be achieved through the application of specialist knowledge, skills and practice.
