About the Role

Are you currently a B3 or B4 looking for part time oppurtunities as a B4 community falls support worker?

Do you have a drivers license and would like to visit patients in their own homes and provide holistic assessment and

care to patients who have fallen or at risk of falls?

Do you have interest in faciliatating exercise classes and providing education to patients?

To respond quickly to all referrals for patients who have fallen or at risk of falling, register and triaging the

referral in accordance to the KPI
