Community Health Nurse - Greenwich
About the Role
MAIN RESPONSIBILITIES To Support the delivery of the Local Health Visiting offer, including the Healthy Child Programme.
To work as an independent practitioner within a skill-mixed team. To support staff as needed/required. To undertake
delegated work in a timely manner. To help identify children and/or families with additional needs, and refer to other
agencies/teams for support and intervention as appropriate. Provide health advice and signpost children and their
families to alternative resources within the local community. To undertake developmental assessments and 6-8 week
reviews when trained and competent to do so. To deliver health promotion activities and targeted interventions to
