Community Health Nurse

About the Role

Build effective relationships with other agencies and disciplines, and work in partnership to promote health and

wellbeing of communities, families, and individuals, and to act as an advocate where appropriate. *To follow local and

national policies and procedures with regards to safeguarding children. To use skills to analyse information to identity

and review families causing concern and escalate to the Health Visitor to agree a plan. *Participate in the attainment

of national and local public health targets and optimise opportunities for health promotion through education and

practice in a community setting. *To promote vaccination programmes and other public health activities as required
