About the Role

We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver

a range of quality care services to patients visited by the Community Response Hubs based at either Chester, Tarporley

or Ellesmere Port. The team visits patients and their carers of all the Integrated Community Care Teams, across Western

Cheshire.

Applicants MUST demonstrate how they will meet the travel requirement for this post.
