About the Role
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver
a range of quality care services to patients visited by the Community Response Hub based at Tarporley Hospital. The team
visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.
Applicants MUST demonstrate how they will meet the travel requirement for this post.
APPLICANTS REQUIRE END OF LIFE CARE EXPERIENCE TO BE CONSIDERED FOR THIS POST.
The hours are 37.5 per week. Consisting of 4 x 9.5 hour split shifts, worked on a rota basis between 08.00 and 22.00
