About the Role
We are looking for enthusiastic, skilled and motivated Community Healthcare Support Workers to join our Quickstart
service across Wandsworth and Merton boroughs to deliver high quality, timely care support with an enablement focus to
patients in the community. We have 1 positions that are permanent.
Quickstart service provides rapid response social care services for patients who are at risk of social care breakdown.
Without a social care package in place these patients would otherwise be admitted to hospital due to an overriding
social care need.
