About the Role

The role of the Heart Failure Nurse within the community has been established in order to meet the needs of people with

heart failure in the Surrey locality is to optimise the management of people with heart failure, reducing unnecessary

hospital admission and providing a link between the GPs and specialist teams. The post holder will offer education and

support to health care professionals, people with heart failure and those who care for them.
