Community Heart Failure Specialist Nurse

About the Role

Manage and facilitate the care of patients with Heart Failure in the community through outpatient clinics and home

visits. Providing specialist assessment for individuals with heart failure and co-ordinate individual care plans to

maximise health outcomes. Use advanced clinical skills and evidence-based knowledge to identify investigation and/or

treatment needs of individuals. Provide medicines management support and advice in accordance with national and local

guidelines. Maintain and further develop expertise within Heart Failure field. To make effective use of learning

opportunities as part of continuing profession development. To evaluate own knowledge, practice and learning needs as
