About the Role

We have a very exciting opportunity to join our integrated Heart Failure Service at Calderdale and

Huddersfield NHS Trust. We are seeking a highly motivated individual with excellent interpersonal

and communication skills. The successful candidate will be responsible for the development of a cohesive management

programme for patients with heart failure in line with local and national guidelines, working across acute, community

and Hospital at Home settings. The service will operate 7 days a week, with a view to extending hours of 8am to 8pm in

the future and requires flexible working within an evolving proactive, dynamic, and committed multidisciplinary Heart
