About the Role
The Hospice Advice & Response Team (HART) plays a vital role in delivering responsive, specialist palliative care across
our community. Operating 24/7, the team provides expert advice, rapid clinical assessment, and urgent interventions to
patients, families and healthcare professionals when it matters most.
As Community Manager (HART) , you’ll be responsible for the strategic, professional and operational leadership of our
HART service, alongside oversight of the Hospice at Home (HAH) team. Working closely with the Head of Community &
Dementia Services and fellow Community Manager, this role ensures seamless, high-quality care across urgent response and
