About the Role
The post holder will collaborate closely with general practitioners, the Hospital at Home team, Urgent Community
Response, specialist community nursing, therapy services, medicines optimisation teams, adult social care, the voluntary
sector, and colleagues across both primary and secondary care. This integrated way of working will ensure that care for
patients and their carers is effectively co-ordinated, seamless, and responsive to individual needs.
