About the Role
This role forms an integral part of the West Essex Care Closer to Home strategy and the Home First approach, both of
which are designed to ensure patients receive safe, appropriate care and treatment as close to home as possible.
Working autonomously as an advanced practitioner, the Community Matron will take a lead role within a designated Primary
Care Network (PCN), coordinating and delivering proactive, evidence-based care across organisational boundaries. The
focus will be on early identification and response to signs of disease exacerbation, ensuring that patients receive
timely interventions aligned with current best practice.
