About the Role

Join Norfolk Community Health and Care NHS Trust as a Community Matron, where you'll provide advanced case management and clinical care for patients with long-term conditions. Responsibilities

  • Assess and provide interventions for patients with long-term conditions.
  • Facilitate early discharge from hospitals.
  • Prevent unnecessary hospital admissions.
  • Collaborate with healthcare professionals for integrated services.
  • Manage a caseload of patients with complex health needs.
  • Educate multi-disciplinary teams on intensive case management.
  • Champion care for individuals with long-term conditions.

Requirements

  • Registered nurse with relevant clinical experience.
  • Strong communication and organizational skills.
  • Ability to work collaboratively in a team.
  • Experience in case management of complex patients.
  • Non-medical prescribing qualification preferred.

Location & Schedule

Position based in Kings Lynn and Coastal PCNs, offering a 7-day service from 8 am to 5 pm.
