About the Role

Responsibilities
- Assess and provide interventions for patients with long-term conditions.

- Prevent unnecessary hospital admissions.
- Collaborate with healthcare professionals for integrated services.
- Manage a caseload of patients with complex health needs.
- Educate multi-disciplinary teams on intensive case management.
- Champion care for individuals with long-term conditions.
Requirements
- Registered nurse with relevant clinical experience.
- Strong communication and organizational skills.
- Ability to work collaboratively in a team.
- Experience in case management of complex patients.
- Non-medical prescribing qualification preferred.

Location & Schedule
Position based in Kings Lynn and Coastal PCNs, offering a 7-day service from 8 am to 5 pm.
