About the Role

Clinical To work across a variety of community settings as part of a multi-disciplinary team. Providing a nursing

assessment and identification of a referred clients mental health needs at home/in the persons own environment, in

circumstances when they have been seen by another member of the multi-disciplinary team. Formulating, implementing and

evaluating collaborative care plans that reflect the needs of the clients/carers over a 24-hour period. Manage own

caseload with a degree of autonomy having responsibility for the nursing care of clients until point of discharge.
