About the Role
The opportunity has arisen to join the Community Mental Health Team at CMHT West in Stockport. The successful post
holder will be joining the team having recently undergone Transformation in line with other Pennine Care CMHT's and NHS
England guidelines.
This role requires staff to undertake the role of Keyworker in the assessment, treatment, and delivery of care to
individuals with severe and complex mental health problems, who meet the criteria for access to the service.
You will ensure that the highest quality care is carried out in accordance with the policies and procedures of Pennine
