Community Mental Health Practitioner, In-reach Role

About the Role

JOB SUMMARY Has continuing responsibility for the provision of skilled interventions to people with mental health

problems in the community. Works within a multi-disciplinary team as part of the Intensive Home Based Treatment Team and

in the role of Senior Mental Health Practitioner for the Mental Health Liaison Service (if applicable to the business

continuity of services). Key Result Areas: Management of team caseload in line with C.P.A. procedures and risk

management protocols. Provision of leadership to the Intensive Home Based Treatment Team in respect of psycho-social

interventions to people and their carers in line with national and local practice standards e.g. NICE Supervision and
