About the Role

This is a Band 6 Community Mental Health Practitioner role working within Older adult services in Tameside. We would

consider a Band 5 to 6 development role for the right candidate.

This role involves providing interventions to identified people in a community setting, with severe and enduring mental

health issues and/or an organic diagnosis. We work with particularly challenging and complex cases.

To work in the community and liaise with in-patient areas assessing and

prioritising patients appropriately. To manage a caseload of patients with complex and challenging mental health needs.
