About the Role

The Support Worker role sits within the Clinical Assessment Team in the South London Partnership (SLP) Complex Care

Programme. This is an exciting time to join the programme as we are expanding our team, working innovatively within

rehabilitation, complex care pathways across South London, including the delivery of Personal Health Budgets. A key

focus to the SLP Complex Care Programme is to ensure people are supported in the least restrictive setting, close to

home and have a personalised outcomes-based support plan that builds on their skills and assets and community

connections.
