About the Role

We are looking for Community Nurses to join our Workforce Solutions team to work "bank" shifts at Provide, in the Mid

Essex area.

You will need to be a driver with access to a car.

The Integrated Care teams work with key stakeholders across Mid Essex to identify adult patients within the community

with long term conditions or short term health care needs.

To achieve successful outcomes the team plans and delivers individual pathways of care involving the client and family
