About the Role

The post holder will manage autonomously as part of a highly motivated team of Case Managers providing a high standard

of care, utilising holistic skills in the assessment, planning and implementing pro-active care needs to a population

with Long Term Conditions in the community settings.

The post holder will work in collaboration with Primary and Secondary care clinicians, social care and mental health

colleagues, to ensure care is patient centred and appropriate to their assessed needs.
