About the Role
The post holder will be responsible for assessment of care needs, development, implementation and evaluation of
programmes of care for patients within the Team Leader’s sphere of care.
The post holder works without direct supervision and is responsible for nursing care for the geographical area including
weekends and evenings.
The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out
of Hospital Care to support West Essex Residents. The post holder will be working as part of the Integrated
Responsibilities
