About the Role
Work as part of the team undertaking responsibilities as negotiated and agreed with District Nurses. Treat all patients
as individuals, respecting their privacy and dignity at all times Assess individual care needs, implement and evaluate
holistic programmes of care Involving, supporting, informing and educating family/carers Take every opportunity to
promote health and wellbeing of the patient Develop effective communication and liaison systems Maintain clear and
comprehensive, signed and contemporaneous records according to SDHC procedures Facilitate/undertake training and
professional updating for team and students Maintain, develop and monitor standards of evidence based nursing care
