About the Role
Duties with the practice includes: Conduct Structured Medication Reviews (SMRs) and post discharge medicines
reconciliation. Manage repeat prescribing, eRD, formulary adherence, and cost effective prescribing. Run clinical
sessions (e.g., hypertension treatment escalation, asthma/COPD optimization, polypharmacy, deprescribing). Support QOF,
IIF/PCN objectives and quality improvement projects/audits. Provide clinical advice to GPs, nurses, HCAs and care
coordinators; contribute to MDT huddles/meetings. Handle medication queries from patients and care homes; deliver
patient education. Implement prescribing safety measures (e.g., high risk drugs monitoring, MHRA alerts). Develop and
