Community Physical Health Nurse
About the Role
To coordinate the physical care of service users carrying out assessment planning, implementation and evaluation of
their care under the supervision of the line manager. To ensure that the service users physical health status is
highlighted and proactively followed up where needs exist by the most appropriate care team. Ensuring that all relevant
information with regards to a service users care and treatment is documented accurately and legibly into their health
record. Supervise and monitor the entries made by junior team members, through performance management and audit. To
liaise significant others when completing the assessment and developing a care plan. To direct other staff in order to
