About the Role
JOB DESCRIPTION
The practitioner will work as a member of the core Physical Health Monitoring Team across the trust. They will work
within a matrix organisation promoting effective inter-disciplinary and inter-agency relationships and acting as a link
between clients, professionals and other services to ensure continuity of care. As a member of the physical health
monitoring team, the post-holder will provide skilled assessments and provide evidenced based brief interventions to
clients in relation to their physical health needs, liaising with relevant agencies and carers. Mental Health services
