About the Role

Exciting opportunities have arisen for new Community Rehabilitation Assistants to join Poole Intermediate Care Service.

The chosen candidates will work as part of a Multi-disciplinary Team caring for patients with an illness in their own

homes to prevent hospital admissions and also to facilitate complex discharges from hospital. You will work alongside

qualified Occupational Therapists, Physiotherapists, Nurses, Therapy Technicians and wider locality teams. Good

communication skills and a caring attitude are needed.

This is a full time post of 37.5 hours per week. Shift working is required as the service operates 365 days a year 08.00
