About the Role
JOB OVERVIEW
The post holder will collaborate with the Optimizing Care Lead and the broader Integrated Community Nursing team to
provide high-quality care while fostering self-management among patients within the existing caseload. Responsibilities
include assessing and determining the appropriate level of support required for current patients, utilizing
motivational, coaching, and negotiation skills to promote independent management of healthcare needs and facilitate
timely discharge from the service.
