About the Role

The Community Staff Nurse will work within the District Nursing triage team, supporting patient discharge plans and ensuring high standards of care in the community.

Responsibilities

  • Support discharge plans for seamless transitions
  • Maintain high clinical practices and care standards
  • Deliver evidence-based wound care
  • Provide excellent End of Life care
  • Evaluate care programs for specific patient groups

Requirements

  • Registered Nurse with a valid NMC pin
  • Experience in community nursing preferred
  • Strong communication and teamwork skills
  • Ability to work across various localities
  • Commitment to continuous professional development

Compensation

Band 5 salary, to be discussed based on experience and qualifications.

Location & Schedule

37.5 hours per week across Tameside and Glossop localities.
