About the Role
The Community Staff Nurse will work within the District Nursing triage team, supporting patient discharge plans and ensuring high standards of care in the community.
Responsibilities
- Support discharge plans for seamless transitions
- Maintain high clinical practices and care standards
- Deliver evidence-based wound care
- Provide excellent End of Life care
- Evaluate care programs for specific patient groups
Requirements
- Registered Nurse with a valid NMC pin
- Experience in community nursing preferred
- Strong communication and teamwork skills
- Ability to work across various localities
- Commitment to continuous professional development

Compensation
Band 5 salary, to be discussed based on experience and qualifications.
Location & Schedule
37.5 hours per week across Tameside and Glossop localities.
