About the Role
We are advertising for a Community Staff Nurses (22.5 hours per week) to work within the Integrated Community Team based
in Saffron Walden Community Hospital and covering the North Uttlesford geography supporting supporting the Care Closer
to Home model.
The post holder will be working as part of the Integrated Neighbourhood Teams supporting the North Uttlesford Integrated
Neighbourhood Team with a focus on prevention of admissions and facilitating hospital discharges
