Foundation Trust is an integrated community and acute trust providing high quality service to residents of City and
Hackney borough.
We are seeking an enthusiastic Band 5 Community Staff Nurse to join the Clinical Assessment Team (CAT), based within the
ACN Single Point of Access.
• The successful candidate will undertake holistic initial assessments on individuals referred in to the ACN service.
