About the Role

Act as an autonomous practitioner in accordance with and upholding the NMC Code of Conduct and Scope of Professional

Practise for Nurses, Midwives and Health Visitors, providing a holistic approach to health by: Completing a full

assessment of the physical, social, emotional and environment needs of people with long term conditions and frailty.

Being pro-active in identifying health needs in this client group. To advise and inform the patients and their carers

about health issues to support with a greater sense of self-reliance and independence.
