Community Stroke Nurse

About the Role

The post holder will be a member of a specialist interdisciplinary rehabilitation team working with patients with acute

and long term needs in the community that are neurologically related. The post holder will be expected to provide for

the client group by assessing and managing health and rehabilitation needs liaising with primary care, the acute sector,

allied health professionals and social care. Health promotion around stroke and the provision of information and

education in the community is a significant aspect of the nursing role Main Responsibilities a) To plan, develop,
