About the Role

To work within the Community Stroke Service on a Bank contract providing specialist nursing assessment and treatment for

patients following a diagnosis of stroke.

This role will assist in the assessment and treatment of patients with newly diagnosed stroke, within the West Norfolk

locality, who have been discharged.

This role will sit within the West Norfolk community stroke service maximising patients optimal level of independence.

• To in-reach into The Queen Elizabeth Hospital Stroke services where necessary and work collaboratively with the wider
