About the Role

We have an exciting opportunity for a Stroke Community Rehabilitation Support Worker to join our established Integrated

Community Stroke Service to deliver rehabilitation into various community settings. The role of Stroke Community

Rehabilitation Support Worker in the Multidisciplinary Community Stroke Team would be to provide high quality holistic,

evidence based care to patients / service users that is responsive, effective and efficient. You would work with

Occupational Therapists, Physiotherapists, Speech and Language therapists, Clinical Psychologist, Assistant

Practitioners and Rehab Support Workers.
