About the Role

We have a rare and exciting opportunity for suitability experienced and enthusiastic Nurse or Therapist who has

appropriate drive, knowledge and leadership skills ideally in a community setting. Our multidisciplinary community

stroke team provide rehabilitation support to patients in their own homes and bed bases across the footprint of CCICP

utilising a Home First approach and proportionate care working closely with acute and community colleagues.

As Team Leader you will be based with the Community Stroke Team at Infinity House where you will be a visible and

present leader who is professionally responsible and accountable for delivery of quality, safe patient care by
