THE COMMUNITY SUPPORT SERVICE SUPPORTS PEOPLE TO REGAIN THEIR INDEPENDENCE THROUGH REABLEMENT. WE ARE A CQC REGISTERED

SERVICE THAT IS RATED OVERALL 'OUTSTANDING'.

OUR REABLEMENT TEAM WORKS WITH INDIVIDUALS TO INCREASE THEIR ABILITY TO LOOK AFTER THEMSELVES FOLLOWING ILLNESS OR

INJURY AND TO REBUILD THE SKILLS AND CONFIDENCE TO HELP THEM LIVE AS INDEPENDENTLY AS POSSIBLE. WE ARE WORKING CLOSELY

WITH COLLEAGUES AND PARTNERS IN HEALTH TO ENSURE THAT WE HAVE AN INTEGRATED AND COLLABORATIVE APPROACH TO OUR SERVICE.
